I hereby expressly consent to and authorize the Bank (whether acting by itself or through any of its service providers, and whether in automated manner or otherwise), to collect, store and process my application details, personal data and sensitive information about me, information, papers and data relating to know your customer (KYC), credit information, and any other information about me/pertaining to me or not as may be deemed relevant by the Bank (collectively, “Information”).

I expressly consent Bank to share and disclose the Information to service providers, consultants, credit information companies, information utilities, other banks and financial institutions, affiliates, group companies, subsidiaries, regulators, investigating agencies, judicial, quasi-judicial and statutory authorities, or to other persons/institutions/entities as may be necessary in connection with the contractual or legal requirements or in the legitimate interests of the Bank or as per the consent, undertake to process information including by way of storing, structuring, organizing, reproducing, copying, using, profiling, etc. as may be deemed fit by the Bank and for the purposes of credit appraisal, fraud detection, anti-money laundering obligations, for entering into contract, for direct marketing, for cross selling, for developing credit scoring models and business strategies, for monitoring, for evaluating and improving the quality of services and products, or for any purposes as the Bank may deem fit. I declare and confirm that for opening Credit Card Account with your Branch at Axis Bank, my local address, which is to be recorded as my mailing address in the application I am enclosing the permanent address proof alongwith this application. I authorize the Bank to send all correspondence, including all deliverables to me at the aforesaid mailing address. I undertake to immediately inform the Bank in case there is any change in my registered correspondence, but not later than 2 weeks of such change. In case the address submitted by me as proof, undergoes a change, I note to submit the fresh proof of address to the Branch for updation of the records. I hereby agree and undertake to hold harmless and keep Axis Bank fully indemnified against claims and damages, which may be made in respect hereof by reason of Axis Bank relying and acting on this declaration.

I hereby apply for the issue of Axis Bank Credit Card to me and declare that the information included in this application is true and correct and that I am a Resident India/ NRI / Foreign National working in India and that I am eligible to apply for an internationally valid Card. I / We unconditionally agree and accept that, the data provided by me/ us to the Bank during the application process for acquiring such relationship(s)is true to the best of my / our knowledge and belief, and if at any stage of processing the relationship. If it comes to the knowledge of the bank or the bank is of the opinion that I / we have provided any incorrect information and / or fabricated documents, and / or fake documents, and / or documents appearing to have been manipulated they will be treated by the bank as having been manipulated by me/ us. I hereby understand and agree that it is my responsibility to obtain, read and understand the Terms and Conditions related to the Axis Bank Credit Card and those applicable to mobile and Internet Banking Services. If this application is accepted. I hereby undertake to be bound by the Terms and Conditions as may be in force from time to time and use of the Axis Bank Credit Card shall be deemed to be unconditional and irrevocable and irrevocable acceptance of the Terms and Conditions.

Further I agree that all communication pertaining to the add – on card holder shall be addressed to me. I understand that the continuation of the membership of the additional card member will be dependant on the continuation of my membership. I understand and undertake that the usage of the Axis Bank Credit Card shall be strictly for bonafide personal purchase of goods and/or services and in accordance with all applicable laws (including without limitation to any government acts, orders, decrees, guidelines, rules & regulations including foreign exchange control regulations) and in the event of any failure to do so, I will be liable for any action/prosecution or penalty as prescribed. I shall not use the card to purchase anything to resell for commercial or business purposes to derive any financial gains. I shall not be able to claim Input Tax credit on GST levied on this card.

I further understand and agree to the levy of all additional statutory levies, charges, taxes. Service Tax as applicable on all fees, interest and other charges as per the government of India regulations and agree to pay the same. I / We also unconditionally agree and accept, that the bank shall have every right to compare the data available with the bank or provided with, by any other Bank. Financial institution . I authorize Axis Bank Ltd. to increase my credit limit within 6 months of Card Issuance. Axis Bank Internal policies and other terms and conditions will be applicable.

I/ We am/are hereby informed that the Axis Bank Instant Credit Card is a virtual Credit Card that will be issued to me / us for domestic usage only. I / We will also be issued an additional physical Credit Card on this Axis Bank Instant Credit Card .The physical Credit Card issued along with the virtual Credit Card will be sent to my / our Bank communication address as provided in Bank records, within 5-10 working days. Physical Card usage would depend on the usage option selected by me / us at the time of submission of application. Physical Credit Card will be issued by the Bank, subject to Bank policy and applicable issuing terms and conditions. The physical version will be the same product wherein the Credit Card Number will be different and will have two different welcome kits. I / We will be issued a single consolidated e-statement as well as physical statement inclusive of purchases made on both the Credit Cards (virtual as well as physical credit card). I/ We agree that Axis Bank Instant Credit Card will be only valid for domestic usage for Card Not Present (CNP) transactions on Point of Sale (PoS) or, online platforms as well as at those portals which accept the Payment from the Payment Application. I / We  am/are hereby informed that in the event of any failure in the issuance of the Instant Credit Card (virtual), Axis Bank will dispatch the physical Credit Card to my / our communication address.
